Important Forms and Papers

Parent registration for Trail Life USA involves two main components: national registration through the Trail Life Connect portal and local troop-specific forms The national process requires a parent to accept an invitation from a troop leader, create an account, and pay annual dues (typically $40–$42.27 per son).  During this process, parents must complete the Youth Health and Medical Form, which collects emergency contacts, insurance information, and medical history.

Additionally, each troop requires specific local paperwork. Common requirements include a Member Participant Health and Medical Form, a Policies Acknowledgement Form, and sometimes a Troop Registration Form to collect local dues.  Parents are also responsible for providing a copy of their health insurance card and completing any troop-specific waivers, such as those for transportation or high-adventure activities.

  • National Dues: Approximately $40–$42.27 per year, paid via Trail Life Connect.

  • Medical Forms: A Youth Health and Medical Form is mandatory for all youth; a High Adventure Medical Form (signed by a doctor) is required for events over 72 hours or high-exertion activities.

  • Local Forms: Vary by troop but often include policy acknowledgments, local dues registration, and specific activity waivers.

  • Adult Registration: Parents are encouraged to register as Registered Adults, which involves background checks and completing an Adult Member Participant Health and Medical Form.
